The answer is:

Oviraptor philoceratops
   discovered by: George Olsen in 1923.  Its remains consisted of a
partial skull and skeleton, at Shabarakh Usu, southern Mongolia.
The skull was rather badly fractured but preserved in a sandstone nodule.
The skull had many openings in the side and a curious horn-like prong on
the tip of its snout.  So reconstructions show it having a horn.  However,
new material found shows that the horn is just part of the bony nasal
crest which runs from the front of the snout to the back of its head.
